How to select TCM
Currently, more and more Western Doctors, besides Herbalist Doctors, select
TCM to treat patients, because of TCM advantages like credible curative effect,
convenient taking and slight side effect. Still, somebody have little
knowledge on pharmacody of TCM, causing some problems of inefficay even the
deterioration diseases, so more attention should be paid to it.
Many TCM drugs have similar name, but their effect and range of indications
are very different, so, while buying TCM drug, the patients should find out
their effect and range of indications, instead of basing on TCM drug name to
buy it. For instance, people usually consider the drug with name of ginseng
and young pilose antler as tonic, actually, many TCM drug containing these
two medicinal materials are not used for tonification, i.e. Ginseng Restorative Bolus used for arthralgia and cold-dampness entering the collateral channels; Ginseng spleen-strengthening Bolus used for dyspepsia; Bolus of Ginseng, Young pilose antler and Bian used for deficiency of the kidney. Bolus of making kids fatty, which made from malt, medicated leaven, haw, rangoon creeper and areca, does not contain the ingredient like Ginseng and Young pilose antler, but leaves the misleading appearance to people that it can make kids fatty, actually it is used for children's indegestion and paratosis. So it is harmful for people to take the above-mentioned TCM drug as tonic.
The situations are common of different TCM drug with same name and same TCM
drug with different name, because there is no uniform formula and naming
standard, never substitute them casually. For instance, Bolus for Tonifying
the Kidney-qi, made in Shanghai, Hangzhou, Xi'an and Jinan on the basis of
the prescription in Synopsis of Prescription of the Golden Chamber written
by Zhang Zhongjing is also called Bawei Bolus containing yaw, tuckahoe,
cassia twing, dry glutinous rehmannia, oriental water plantain rhizome,
moutan bark, prepared aconite root and dogwood fruit. However, Bolus for
Tonifying the Kidney-qi, made in Tianjin, Tangshan, Shenyan and Harbin on
the basis of the prescription in Recipes for Saving Lives written by Yan
Yonghe of Song Dynasty, is made from the mentioned 8 species medicinal
material and plantain seed and achyranthes root. These two TCM drugs have
some name but different effect and range of indications. The former drug
has marked effect in treating lumbago, difficult urination, sensation of
contracture of the lower abdomen, but the latter one is good for treatment
of inducing diuresis to alleviate edema. So the drug should be taken on
patients' state of an illness discriminatively.

Still, some TCM with different name have similar effect, i.e. Pill of Six
Ingredients with Magical Effects, anti-inflammatory pill and laryngitic
pill are good for treatment of pharyngitis; both pill of lonicera and
forsythia and pill of antelope's horn and forsythia are used to treat
wind-heat type of common.
Selecting drug discriminatively
It is common that different patients suffering from some illness take some
drug and get different result. What's the reason? That's because of curative
characteristic of herbalist doctor- Bianzhengshizhi that means diagnosis
and treatment on the basis of an overall analysis of the illness and
patient's condition. For instance, herbalist doctors think that cold has
two types of wind-cold and wind-heat, and should choose different drug.
Pill of lonicera and forsythia containing lobster sauce, mint, burdock,
bamboo leaf, common reed rhizome, liuorice, forgythia, schizonepeta and
balloonflower, is transferred from powder of lonicera and forsythia in
Detailed Analysis of Epidemic Febrile Diseases. It, holding the effect of
disperse wind and heat, and open the inhibited lung energy and receive
cough, is good for treatment of wind warm, fever and thirst, but can not
be used to treat wind-cold type of common cold with the symptom of externa
wind and cold, fever with chills. While wind-cold type of common cold being
treated with taking Pill or Bolus of lonicera and forsythia, fistrlar onion
soup should be taken as guiding drug. Also, it can be treated with Vitamin C
Pill of lonicera and forsythia or fine Pill of lonicera and forsythia,
because these two drug contain Vitamin C, caffein, haynon and metalid,
and these two drugs can be used to treat not only wind-cold type of common
cold, but also wind-heat cold.
Also, white phoenix bolus of black-bone chicken & pill arresting uterine
bleeding are used to normalize menstruation, however, the former one is
good for the treatment of irregular menstruation with the characteristic
of scanty menstruation, the latter one has marked effect in treating
irregular menstruation with the characteristic of profuse menstruation.
So, these two drugs can not be substituted each other. For same reason,
Cinnabar Sedative Pill & mind-easing tonic bolus with arborviate seed both
are sedative-hypnotic, but, the former one has effect of clearing away the
heart-fire and relieving convulsion and mental strain caused by deficiency
of the heart-fire and excessive cardiac fire, and the latter one has the
effect of tonifying the qi and enriching the blood, being good for the
treatment of insomnia and dreaminess caused by deficiency of the heart-qi
and deficiency of heart-yang. These two drugs can not be substituted each
other, because of their different characteristics of Yin and Yang.

While treating patients, more attention should be paid to their specific
conditions. For instance, when pregnant woman injuries from falls, the
drug with the characteristics of removing blood stasis and invigorating
the circulation of blood, i.e. anti-bruise powder and bolus of dieda can
not be used to treat them, if used, it may causes Ab-abbortion. The cough
patient having heart disease history can not take the drug like powder of
snake bile and dried tangerine peel and tendrilled fritillary and loquat
leaf syrup containing ephedra and ephedrine, otherwise it probably cause
the relapsing of heart disease.
